Lumbar myelography was carried out with the contrast media Amipaque, Dimer X and Myelografin in 10 patients each. Five of the patients treated with each contrast medium were kept in a sitting position after the examination, the others lay flat. Blood levels and excretion were measured up to 24 h. The results are interpreted as follow:

A 14 year old child who had three episodes of subarachnoid haemorrhage, was found by spinal arteriography to have a cervical extramedullary arterial aneurysm combined with an intramedullary arteriovenous angioma. The source of the bleeding, the aneurysm, was excised surgically but the intramedullary arteriovenous angioma was not accessible for operation.
In 34 patients with rheumatoid arthritis we determined the regulatory index OKT4:OKT8 and the suppressor index Δ24:Δ0. There is no correlation between the results of the detection of subpopulations with monoclonal antibodies and of the functional test to determine the suppressor activity.

Cochleomeatal scintigraphy (CMS) and CT or air CT cisternography are today the methods of choice for the detection or exclusion of a neurootologically suspect acoustic neuroma. Whereas CT as a morphological method of investigation provides better preoperative information, CMS is likely to be superior to CT as a functional investigation technique in relapse diagnosis.
The resonance Raman scattering observed in the exciton region of indirect AgBr nanocrystals with mean particle radius 〈R〉 ≈ 5 nm is analyzed. Considering the processes as absorption followed by emission and calculating the size dependent exciton energies by the donor-like exciton model of Ekimov et al. , the 1TO(L) and 2TO(L)&LO(Γ) resonance behavior can be well-reproduced. The analysis of the Raman spectral lineshape based on linear exciton-phonon coupling shows a substantial increase of coupling strength with excitation photon energy. It is attributed to enhanced acoustic and surface phonon interaction of higher exciton states.
Low-temperature exciton spectra are investigated in AgI nanocrystals in a monocrystalline KI matrix. The occurrence of the Z1,2 together with the A and B exciton absorption bands demonstrate that both zincblende (γ) and wurtzite (β) structure nanocrystals have formed. The exciton blue shift implies a case of weak confinement and an average particle radius of about 2.5nm. Size-selective excitation results in line narrowing and reveals detailed structure in emission due to the LO phonon coupling and the fine structure of the A exciton state. From the analysis the short-range electron–hole exchange energy is found to vary between 2 and 7meV with decreasing particle radius. The origin of another structure with size-dependent energy shift of 7–11meV is discussed. A size-dependent scattering of the B exciton state into the A exciton state is observed.

AgI nanocrystals were synthesized in reverse micelles. From X-ray diffraction patterns the nanocrystals were found to exhibit predominantly a zincblende structure. The exciton absorption was blueshifted and inhomogeneously broadened due to spatial confinement and the particle size distribution. “Free” exciton luminescence was weak. Instead, in addition to donor–acceptor recombination, a strong LO multiphonon Raman scattering was observed under resonant excitation in the exciton absorption. An LO phonon energy of 15.4 meV was derived and the incoming and outgoing resonances with the exciton were investigated. All results were consistent with the case of weak exciton confinement.

Unter 1091 Bandscheibenoperationen fanden sich in 124 Fällen epidurale Gefäßanomalien. 76mal kamen diese Gefäßveränderungen kombiniert mit einer Nucleus pulposus-Hernie und 48mal isoliert vor. Histologisch handelte es sich bei 18 untersuchten Fällen überwiegend um venöse Erweiterungen mit und ohne Wandveränderungen, zum Teil auch um echte Angiome. Das Beschwerdebild ähnelt dem des Bandscheibenvorfalles; einige Besonderheiten können jedoch zur Verdachtsdiagnose führen. Röntgenologisch wurden häufig Dysraphien festgestellt. Im Myelogramm war bei jedem dritten Patienten eine isolierte Wurzelscheidenerweiterung nachweisbar. Ossovenographische Untersuchungen führten nur ausnahmsweise zu einer präoperativen Diagnose. Katamnestische Untersuchungen zeigten, daß 50% der Patienten mit isolierten epiduralen Gefäßanomalien und 80% der mit einem Vorfall kombinierten Formen beschwerdefrei wurden. Die häufige Kombination mit lumbosacralen Dysraphien weist auf die kongenitale Entstehung der isolierten Gefäßveränderungen hin, wenn diese auch überwiegend als Varizen anzusprechen waren. Pathogenetisch scheinen mechanische Faktoren, Stauungen und Zirkulationsstörungen bestimmend zu sein und durch chronische Irritation zu sekundärer Wurzelbzw. Wurzelscheidenschädigung zu führen.
To see whether intestinal permeability (IP) predicted relapse in Crohns disease, we measured IP in 72 patients with quiescent Crohns disease using the lactulose-mannitol test. The permeability index (lactulose/mannitol) was significantly higher in patients than in controls (0·046 [SEM 0·005] vs 0·018 [SEM 0·002], respectively). Patients were followed for 1 year after the test. 26 of the 37 patients with raised permeability, but only 6 of the 35 with normal permeability relapsed within 1 year after the test (p<0·001). The sensitivity of the permeability test as a predictor for relapse was 81 %. A significant correlation was found between the value of the permeability index and the probability of relapse (p<0·01). These results show that increases in intestinal permeability precede clinical relapses in Crohns disease and so are an indicator of subclinical disease. The measurement of intestinal permeability may lead to a better understanding of the pathogenesis of Crohns disease.
The aim of this study was to assess sexual function and quality of life (QoL) in patients after surgery for perianal Crohn’s disease. Eighty-eight consecutive patients with perianal Crohn’s disease, operated on at the Medical University of Vienna, completed a self-administered questionnaire including the International Index of Erectile Function (IIEF), Female Sexual Function Index (FSFI), Short Form-12 Health Survey (SF-12), and the Inflammatory Bowel Disease Questionnaire (IBDQ). Patients with a current stoma were excluded from further analysis. The median follow-up time was 104 months (range 3–186 months). Healthy subjects served as controls for each case and were matched by age (±6 years) and gender. Forty-seven (68 %) female and 22 male patients with a median age of 46.5 years (range 18–64 years) were analyzed. Eleven (16 %) patients had simple and 58 (84 %) complex anal fistulas. The median SF-12 physical health score of the patients was significantly lower (47.9 (range 25.5–57.2)) than that of the controls (54.3 (range 34.6–61.8); p = 0.03). Not surprisingly, the median total sore of the IBDQ of the controls was significantly better than that of the patients (controls: 188.5 (range 125–206.5), patients: 157 (range 60–199.5); p < 0.0001). Analysis with the multiple logistic regression test showed that type of operation, >1 perianal fistula opening, and active Crohn’s disease were independent risk factors for a worse IBDQ (p = 0.03, p = 0.015 and p < 0.0001). Interestingly, the median FSFI and IIEF score were not found to be significant different in any domain. QoL but not sexual function is significantly influenced by surgery for perianal Crohn’s disease.

When the cervical spine is involved in primary chronic polyarthritis, this leads in 6–8% of cases to an atlantoaxial dislocation or to luxations in the subaxial region. This may in turn lead to spinal cord compression and to an intermittent vertebral artery insufficiency. The clinical and radiological findings are described in 4 cases and the symptomatology discussed with the aid of the literature. The recognition of such complications in primary chronic polyarthritis is important, since neurosurgical measures for the stabilization of the cervical spine can produce impressive remissions.
Immunofluorescence analysis of CD27 expression by CD4 lymphocytes from the peripheral blood of healthy humans or rheumatoid arthritis (RA) patients and from the synovial fluid (SF) of RA patients was carried out, along with the estimation of cytokine gene [interleukin (IL) 2, IL-3, IL-4, IL-5, IL-6, IL-6R, IL-10 and interferon-γ (IFN-γ)] expression in these lymphocyte subsets by RT-PCR. Although no differences in CD27-positive and negative peripheral blood CD4 cell subset distribution were revealed, marked differences in IL-3, IL-4, IL-5 and IFN-γ mRNA expression were detected between these lymphocyte subsets and between control and disease states. These results showed that phenotyping of different cell subsets in disease cannot provide adequate information about lymphocyte functional status. To estimate differences in cytokine gene expression, CD4 lymphocytes from the peripheral blood and SF of RA patients were compared. In both cases, mRNAs for IL-2, IL-4, IL-10 and IFN-γ were detected, but CD4 cells from SF failed to express detectable levels of IL-5 mRNA despite our findings of a CD27- cell accumulation within the synovial population of CD4 lymphocytes. These are the first data to demonstrate that expression of the IL-5 gene in RA SF CD27- lymphocytes is down-regulated and that IL-5 disregulation in RA cannot be ruled out.
The enantiomers of ciprofibrate may be achieved by enantioselective HPLC separation of its methylesters using a OD — Daicel column. Ciprofibrates (racemate and both enantiomers) bind to oxidized cytochrome P-450 in rat liver microsomes according type II like aniline or most probably as inversed type I, but less pronounced and with a general shift to the left. Ethylmorphine N-demethylation, ethoxycoumarin and ethoxyresorufin O-deethylation are all inhibited by the ciprofibrates, most effectively ethoxyresorufin O-deethylation by S(-)-ciprofibrate even in μM concentrations. Microsomal luminol and lucigenin amplified chemiluminescence indicating the formation of reactive oxygen species, microsomal hydrogen peroxide formation and NADPH/Fe stimulated lipid peroxidation were inhibited in a concentration dependend manner in concentration ranges between mM and μM. This might be due to distinct scavenger activities of all 3 compounds: the zymosan stimulated chemiluminescence of whole blood was completely inhibited in mM concentrations and influenced significantly down to concentrations of 10 μM, whereas burst and phagocytosis tests with human polynuclear leucocytes were not influenced.
Die Entwicklung des nicht-ionisierenden wäßrigen Kontrastmittels Metrizamide erlaubt erstmals seine Anwendung im gesamten Spinalkanal. Eine der Voraussetzungen dafür — die fehlende Reizung des Rückenmarkes — ist für dieses Kontrastmittel gegeben. Damit verbunden gelingt eine ausgezeichnete und bislang mit anderen Kontrastmitteln nicht bekannte Detailerkennbarkeit im Zervikal- und Thorakalbereich. Der rasch einsetzende Verdünnungseffekt macht für Untersuchungen der Zervikalregion jedoch eine andere Applikations- und Untersuchungstechnik erforderlich (Punktion zur Kontrastmittelinjektion in Höhe von C 1/2). Für die Untersuchungen des thorakolumbalen Überganges und der Thorakalregion sind fraktionierte Eingaben zwischen den Kippungsvorgängen sinnvoll. p ]Wie bisher bei jedem wasserlöslichen Kontrastmittel, kann es auch bei Verwendung von Metrizamide zum Auftreten epileptischer Anfälle kommen. Gemessen an dem großen Untersuchungsgut skandinavischer, belgischer und holländischer Autoren, ist diese Komplikation seltener als 1%. Zur Prophylaxe sollte vermieden werden, daß das Kontrastmittel in höherer Konzentration bei der Applizierung oder später in den intrakraniellen Raum gelangt. Nach Literatur und eigenen Erfahrungen entsprechen die postmyelographischen Beschwerden bei Verwendung von Metrizamide in etwa denjenigen anderer wäßriger Kontrastmittel. Zum Teil werden sie auch in einem höheren Prozentsatz angegeben. Im eigenen Untersuchungsgut waren sie bei zervikaler Applikation geringer als bei lumbaler. Dies kann mit als Hinweis dafür genommen werden, daß die Ursache hierfür in einem Liquorunterdrucksyndrom und nicht als kontrastmittelspezifisch anzusehen ist. In Übereinstimmung mit anderen Autoren sind wir der Ansicht, daß die Myelographie mit Amipaque® neue und verbesserte diagnostische Möglichkeiten bietet. Dies gilt insbesondere für den Zervikal- und Thorakalbereich des Wirbelkanales.
Ten women and 10 men consumed 50 g daily of probiotic sausage (Lactobacillus paracasei LTH 2579) over 5 weeks, after a control period with conventional sausage. Four venous blood samples were taken: at the start of the study, at the end of each period, and after 2 weeks of the probiotic sausage period. Stool samples were collected on the 3rd and 9th day of defined diet at the end of each period and during a washout post-period. Total cholesterol, HDL and LDL cholesterol, triacylglycerides, antibodies against oxidized LDL, and some immunological parameters were analysed. In faecal samples, there was a statistically significant increase in the numbers of L. paracasei LTH 2579, but not in all volunteers. There was no significant influence of the probiotics on serum concentration of different cholesterol fractions and triacylglycerides. At the end of the intervention with probiotic bacteria, a significantly higher titre of antibodies against oxidized LDL was measured. The changes in immunological parameters were within physiological variation. The values of CD4 (T-helper)-lymphocytes increased after 2 weeks of supplementation with L. paracasei, but decreased to the basic value by the end of the study. A significantly positive correlation between the numbers of L. paracasei in the stool and the percentage of cells expressing CD4 was found. The expression of CD54 (ICAM-1) on lymphocytes decreased significantly after consuming probiotic sausage. The phagocytosis index of granulocytes was significantly enhanced at the end of the probiotic period as compared with controls. These findings provide evidence that L. paracasei LTH 2579 applied via sausage modulates various aspects of host immunity.
An instability of the MnO4- molecular ion in KI is reported that occurs under intense resonant excitation in the 1A1 to 1T2 optical transition of the impurity. The intensities of the resonantly excited vibrational Raman lines and the absorption strength are used to monitor the time dependence of this effect which is investigated for different temperatures and excitation powers. A physical model is discussed that involves ionization of the molecular ion and qualitatively accounts for the experimental observations.

In contrary to TNF-α and IL-6, PCT differs in all stages of systemic inflammation proportional to severity in accordance with the consensus conference definitions. PCT values are helpful in newly and severely infected patients, primarily of bacterial cause. After endotoxin injection PCT increases shortly after TNF-α and before IL-6 (1). Hence PCT might serve as an early and dependable indicator in patients suffering from severe infection/systemic inflammation. The results have yet to be confirmed by greater sample sizes.
Resonant Brillouin scattering experiments have been performed in the 1S exciton region of ZnSe layers grown on GaAs substrate. Information about the biaxial strain in the ZnSe epilayer, the exchange splitting δ, and the longitudinal-transverse splitting ΔLT can be obtained by comparing the experimental Stokes shifts with those calculated from the dispersion of the 1S exciton-polariton in the strained crystal. For these calculations we used a Hamiltonian which contains the kinetic energy of the exciton center-of-mass motion, the electron-hole exchange interaction and the strain-induced effects. The polariton dispersion is calculated using a multi-component oscillator model. We find a quantitative agreement between the calculated and experimental Stokes shifts for a biaxial strain of +0.03% and exchange parameters δ=0.4 meV and ΔLT=1.2 meV.
A study is reported of photoinduced bleaching in the resonant Raman (RR) scattering spectra of KI:MnO
crystals under laser excitation of the MnO
centers in resonance with the 1
2 optical transition. The bleaching effect is found to exist for all RR lines and is studied in detail as a function of time, temperature, and laser excitation power for the ν
1 line. A reversible and an irreversible process are observed, whose contributions to the total effect depend on temperature. It is established that the decrease in RR intensity is accompanied by a decrease in integrated impurity absorption. The observed photochromic effects are attributed to photoinduced instability of the MnO
ions. The results find qualitative interpretation within a three-center model including the following photoelectronic processes: photothermal ionization of the permanganate, electron ejection from the excited state of the impurity center into the conduction band, and electron capture by traps of two types, with subsequent trap reactivation. The intensity distribution in a multiphonon RR scattering spectrum obtained at 5 K is used to find the main electron-phonon interaction constants. The introduction of the permanganate ion is found to excite a local lattice vibration, which is observed both near the exciting line and in combination with the fully symmetric intramolecular vibration of the impurity ion itself.
